AP Government careful examination of the matter hereby approve the revised format of Family Members Certificate in place of the earlier format prescribed in the G.O. 237 read for issuance of Family members Certificate to the Government Employees/General Public etc., for all purposes by then Tahsildars.

ORDER:
1. In the reference 3rd read above, the Deputy Accountant General (Funds), O/o the Accountant General of Andhra Pradesh and Telangana has informed that the GPF dues are paid to the deceased eligible family members. There is no column provided in the certificate prescribed in G.O.Ms.No. 237, Revenue (Ser.II) Dept., dated: 30-06-2015 for recording marital status of the family members that causes delay in payment of GPF dues and requested to provide a separate column in the Family Member certificate for furnishing marital status of the family members to facilitate early settlement of GPF dues.
2. In the reference 4th read above, the Special Chief Secretary and Chief Commissioner of Land Administration, A.P. Hyderabad while proposing for addition of new column in the Family Members Certificate ahs requested to issue early orders.
3. Government careful examination of the matter hereby approve the revised format of Family Members Certificate in place of the earlier format prescribed in the G.O. 2nd read above for issuance of Family members Certificate to the Government Employees/General Public etc., for all purposes by then Tahsildars.
4. The revised model format of family members certificate to be issued by the Tahsildars concerned is annexed to this order.
5. The Special Chief Secretary and Chief Commissioner of Land Administration, Andhra Pradesh, Hyderabad shall communicate this order to all Heads of Departments concerned, the District Collectors and Tahsildars in the State.
