The Andhra Pradesh School Education Department has released the AP Mega DSC Teachers Forms 2025 Joining Report and Required Forms for newly selected teachers. Every teacher appointed through the DSC 2025 recruitment process must submit a set of official forms at the time of joining. These forms include the Joining Report, Physical Fitness Certificate, Revised Attestation Form, HRMS/CFMS ID Proforma, PRAN Application, Annexure S5 Covering Letter, APGLI Proposal Form, and PAN Application (Form 49A). Teachers can now download all the required forms in one PDF bundle for easy submission.

DSC 2025 Joining Report

The AP DSC 2025 Joining Report is the first document every newly appointed teacher must submit to the concerned Head of the Institution or MEO. It includes personal details such as name, post, school name, date of joining, and a declaration confirming the candidate’s acceptance of duty. Along with this form, candidates must attach educational certificates, ID proofs, and other required documents.

  • Posting Order (2 copies)
  • Educational Certificates (Original and Xerox)
  • SSC Certificate (Proof of Date of Birth)
  • Passport-size Photo
  • Physical Fitness Certificate (from Government Medical Officer)
  • Attestation Forms (4 sets)
  • Aadhaar, PAN, Bank Passbook copies
  • List of Family Members and Hall Ticket Xerox

Physical Fitness and Health Certificate

The Physical Fitness Certificate must be obtained from a Government Medical Officer. It certifies that the candidate is medically fit for teaching duties. The form records height, weight, chest measurement, and overall health condition. Teachers must submit this certificate before joining the service.

  • Height, Weight, and Chest measurements
  • Vision and physical condition
  • Identification marks
  • Signature and stamp of a medical authority

Revised Attestation Form

The Revised Attestation Form is a crucial verification document required by the District Education Office (DEO). It collects information about residence, education, employment, family details, and character references. Candidates must fill it carefully in their own handwriting and submit four copies through the HM or MEO.

Important Sections:

  • Personal and family details
  • Educational qualifications and previous employment
  • Address and residence history for the last 5 years
  • Referees (two persons not related by blood)
  • Declaration and attestation by a Gazetted Officer

Regular Employee Proforma (HRMS/CFMS ID)

All newly recruited teachers must obtain a unique HRMS/CFMS ID through their DDO office. The Regular Employee Proforma includes details such as personal data, department code, payroll area, and DDO authentication. This form enables teachers to receive a salary through the Comprehensive Financial Management System (CFMS).

  • Aadhaar and PAN card copies
  • Bank account details and the passbook first page
  • Appointment Order (ink-signed copy)
  • DDO authentication via biometric verification

Application for Allotment of Permanent Retirement Account Number (PRAN)

The PRAN Application Form is essential for enrollment under the National Pension System (NPS). All permanent teachers must have a PRAN to manage their pension contributions digitally. The form includes personal, employment, and nomination details and is verified by the DDO before being submitted to the NSDL CRA.

PRAN Form Sections:

  • Personal and contact information
  • Employment details with DDO and PAO codes
  • Nominee details and percentage share
  • Pension Fund Manager and Scheme Selection
  • Declaration and DDO attestation

Annexure S5 – Covering Letter for Subscriber Registration

The Annexure S5 Covering Letter must be submitted along with all PRAN registration forms. The DDO certifies and declares the total number of subscriber forms enclosed for submission to NSDL CRA. Each covering letter should include not more than 50 words.

APGLI Application Form (Proposal Form)

The APGLI (Andhra Pradesh Government Life Insurance) Proposal Form is mandatory for all government employees. This insurance policy provides life coverage during service and savings upon maturity. The applicant must declare their health status, nominee details, salary, and premium details in the form, which is certified by the DDO.

  • Signature of DDO and Employee

Application for Permanent Account Number (Form No. 49A)

Teachers who do not have a PAN must apply using Form 49AThe Income Tax Department issues this form for Indian citizens and includes personal, parental, and address details. It is mandatory to quote PAN for salary, tax deduction, and financial transactions in government service.

Documents Required:

  • Proof of Identity – Aadhaar, Voter ID, or Passport
  • Proof of Address – Electricity Bill, Ration Card, or Aadhaar
  • Proof of Date of Birth – SSC or Birth Certificate
  • Recent Passport-size Photograph
