SAMP-CBA OMR Bundle Slip 2025 AP Self Assessment Term 2 (SAMPT2) Model Paper OMR Submission process for 2025 is a crucial step in the assessment workflow for Classes 1st to 9th. Bundle the OMR sheets subject-wise and class-wise in proper sequence.
Attach a Bundle Slip containing the following:
- School UDISE Code
- Class & Subject
- Total OMR Sheets Count
- Teacher Signature & Date
Submit the OMR Bundle to the Mandal Resource Centre (MRC) or designated collection point as per the official schedule. SAMP-CBA OMR Bundle Slip
Maintain a record of submission in the school for audit/reference.

Instructions for Handling Student Attendance and OMR Sheets:

If a student attends even a single subject, they must be considered present.
In the OMR sheet, bubbling should be done only for the subject the student attended.
The remaining subject fields should be left blank — do not mark an “X” or write “ABSENT”.
For used OMRs, do not use separate covers for each class.
- Use one cover for Classes 1 to 5.
- Use another cover for Classes 6 to 8.
- Only if sheets don’t fit in one cover, a second cover can be used.
Under no circumstances should ABSENT OMRs be mixed with USED OMRs.
All ABSENT and UNUSED OMRs must be kept in a separate cover and sent to DCEB (District Common Examination Board).
Ensure that Annexure-I for the respective school is placed inside each cover and one copy is also pasted on the box.
Mandal Education Officers must collect Annexure-II from each school and submit:
- MANDAL ABSTRACT Annexure-II (Excel Sheet)
- USED OMR covers (class-wise) to the DCEB office without fail.
Final Dispatch to DCEB:
- One box containing USED OMRs for Classes 1–5
- One box containing USED OMRs for Classes 6–8
- One box containing UNUSED and ABSENT OMRs
