Upload Include School Details, Personal Details, Addresses, Patient Details, Hospital Details, and proof of support in the Claim Form.
To complete the bill number and date in documents, you must upload the Discharge Summary, Essentiality Certificate, Emergency Certificate and Dependent Certificates.
Click on submitting Submit Button and Next Display successfully Submitted.
It should be printed and sent to the CSE by the DDO to the Medical Reimbursement Claim Application.
