A medical certificate confirming your good health, which must be no more than six months old. - Legalized copies of your degrees, diplomas, or certificates that are less than two months old and have been translated into English or Czech a legalised translation into Czech by a sworn translator/interpreter entered into the Czech Republic’s list of experts and interpreters
- A legalised copy of a Supplement to Diploma or a list of completed lectures and examinations, including their scope and grades (marks) obtained through a legalised translation into Czech.
